The enrollment packet is completed by the Father Advocate/Case Manager during the initial participant enrollment and Fatherhood Planning meeting.
This packet includes forms 2A – Enrollment Checklist; 2B – Program Status Form; 2C – Application for Services; 2D – Father-Child Interaction Form—Baseline; and 2E – Service Needs Summary.
Additionally, the Father Advocate/Case Manager and the participant should fill out the Fatherhood Plan form together at the time of enrollment:
