Performance Measurement Forms (for use at the St. Cloud FATHER Project site):
Performance Measurement Forms are typically measured at baseline, and then upon completion of 12 parenting groups (some forms are administered only at the conclusion of the 12 parenting groups).

• Parenting Class Performance Measurement Retrospective Survey, English (This form is administered upon enrollment and the again upon completion of 12 parenting groups (all sites in a common version—must submit for grant tracking)
• Parenting Class Performance Measurement Retrospective Survey, Spanish 
• Father-Child Interaction Forms (Time 2) available in English and Spanish, this form is administered upon completion of 12 parenting groups. (all sites in a common version—must submit for grant tracking)
• Employment Readiness Training Retrospective Survey, English (this brief survey is administered upon completion of the ERT training.  (all sites in a common version, must submit for grant tracking)
• Employment Readiness Training Retrospective Survey, Spanish
• FATHER Project Enrollment and Benchmark Completion Form (Father Advocates / Case Managers complete this form immediately when a participant completes one of the identified FATHER Project performance measurement benchmarks).  (all sites in a common version)

Employment Service Forms

• Individualized Placement Plan (all sites in a common version)
• Placement and Advancement Verification Form (all sites in their own version)
• Employment Retention Tracking (all sites in their own version)
