All new and existing participants must sign the appropriate consent form or packet below in order to be tracked in the PACTIS system:

For existing FATHER Project participants:

• Consent form for study participants (English) – should be signed at enrollment
• Consent form for study participants (Spanish) - should be signed at enrollment
• Consent packet for participants being grandfathered in (no RA)
• Consent packet for NEW Dakota County and Hmong-American Partnership participants
• Consent packet for NEW African Community Services participants
• Consent packet for NEW Division of Indian Work participants
